We need your help…
You work most closely with CalWORKs participants and understand
their transportation needs. By using the new transportation forms,
you can help us identify how to develop ideas for long-term transportation
improvements for CalWORKs participants.
New information about the Transportation Forms:
1. Copies are available in SSA offices or by
calling the Job Access Program at OUTREACH at 436-2865.
2. They are now available in Spanish and Vietnamese.
3. There will be a free gift selected at random from those who
submit the Tracking Forms!
1. What are they?
There are several forms available to help you help your customers
with their transportation needs:
· My Transportation Plan is a planning tool for use by participants to plan their routes and modes of transportation to work, training, childcare or other work-related activities. Customers use it to identify where they need to go and call VTA for help with planning the best way to get there. The result is a personal transportation plan to help think ahead about transportation. (Temp SC 39)
· Guaranteed Ride Home Application forms are the first step to enrolling in the program. The GRHP offers enrolled participants up to 48 free rides, provided by OUTREACH, to meet urgent transportation needs. Interested CalWORKs customers should complete the forms and submit them to OUTREACH.
· Transportation Feedback Forms capture detailed information about specific transportation obstacles - alternatives that are not currently available but could help job-ready individuals obtain and/or retain employment. This information will be shared with transportation providers like VTA to help identify options that may better meet the needs of CalWORKs customers. (Temp SC 28)
· Referral and Tracking Forms are our way of documenting tha CalWORKs participants have been assisted and supported with the provision of transportation resources. We need to report this information back to our funders, the Federal Transit Administration and the Santa Clara County Board of Supervisors. (Temp SC21)
2. Spanish and Vietnamese Forms
Several resources have been translated into Spanish and Vietnamese: the Transportation Resource Guides, "My Transportation Plan" and the GRHP application. For copies of this information in Spanish, please call Mario Urtecho at OUTREACH at 408-545-5563 (urtechom@ssa.co.santa-clara.ca.us). 3. GIFT!
