TRANSPORTATION PROVIDERS

Transportation services for Santa Clara County residents 65 years of age or older. Services include subsidized monthly public transit passes, individualized transportation plans, ADA Paratransit Service Rider's Fares, and door-to-door transportation. Please call for eligibility requirements and enrollment forms.

Door-to-door transportation in a lift-equipped van to grocery shopping in the Palo Alto/Stanford area, Senior Health Day Program in south Palo Alto and La Comida’s weekday lunch program at the downtown Senior Center. Transportation to La Comida and grocery shopping is limited to residents of Palo Alto and Stanford. The service area covered for participants attending the Senior Day Health Center includes Palo Alto, East Palo Alto, Stanford, and parts of Los Altos and Mountain View. No fee for transportation to La Comida lunch program. Fees for transportation to Avenidas Senior Day Health Program and grocery shopping vary.

Volunteer drivers provide rides to ambulatory seniors anywhere they want to go during working hours on weekdays. Fees for this service vary depending on the distance traveled.
Catholic Charities Daybreak Respite & Caregiver Support Program - (408) 238-7252
Volunteer-based, escorted transportation to medical appointments and grocery shopping. Requires an initial assessment by a nurse to determine eligibility for program services. Service area is Santa Clara County, from Mountain View to Gilroy. Fee: $150.00 per year.
Heart of the Valley – Escorted Transportation - (408) 241-1571
Heart of the Valley will take seniors to medical appointments, shopping, errands, to hospitals to visit friends and family, and will escort seniors on outings. Serves residents of the cities of Campbell, Cupertino, Los Gatos, Monte Sereno, Santa Clara, Saratoga and Sunnyvale. Suggested Donation: $2.50 plus mileage at $0.25 a mile.
Mexican American Community Services Agency (MACSA) - (408) 928-1155
Limited transportation services for program participants living in MACSA’s service area.
Yu-Ai Kai, Japanese-American Senior Center - (408) 294-250
Limited transportation services to and from Yu-Ai Kai’s center.
The Terraces of Los Gatos - (408) 356-1000
Transportation provided is limited to 8 one-way rides per month. Applicants must qualify for OUTREACH’s Paratransit Program and be a resident of Los Gatos, Saratoga or Monte Sereno.

Private Providers of Medi-Cal Transportation. Non-emergency medical trips for Medi-Cal recipients.
CITY/COUNTY TRANSPORTATION RESOURCES

City of Santa Clara - Santa Clara Senior Citizens Center - (408) 615-3170
Subsidizes rides for ADA. Paratransit Program eligible seniors and disabled adults through OUTREACH’s paratransit program to attend the senior nutrition program. Additionally, City of Santa Clara provides funding to OUTREACH to cover a portion of the client fare for eligible City of Santa Clara residents enrolled in the ADA Paratransit Program.
City of San Jose and County of Santa Clara
Provides subsidized rides for ADA. Paratransit Program eligible seniors and disabled adults through OUTREACH’s paratransit program to attend various Senior Nutrition Program sites in Campbell, Gilroy, Los Gatos, Morgan Hill and San Jose. Additionally, city-owned vans provide free transportation from senior’s homes to the following Senior Centers: Alma, Cypress, Gardener, Hank Lopez, Iola Williams, Southside, St. James and the Willows.
