OUTREACH is a United Way Agency, incorporated as a nonprofit agency in 1979. OUTREACH arranges transportation and hands-on assistance for more than 10,200 frail seniors and disabled adults in Santa Clara County. Since 1973, OUTREACH has freed families and friends from worrying about how their loved ones will get to the doctor, go shopping, or fill out benefit forms. OUTREACH continues to provide vital services to keep people living as independently as possible.
Many of the people needing help are older and live on a low, fixed income with multiple challenges to overcome-debilitating arthritis, vision or hearing loss, life-threatening illness, severe memory impairment, or diminished strength. Others are younger but face mobility limitations, or do not have the mental acuity necessary to manage on their own.
The 65-plus population in Santa Clara County is expected to triple by 2020. The same medical and technological advances that allow extended life-spans for older adults are also allowing disabled people of all ages to live longer. OUTREACH is working hard to build systems and support base to keep up with these growing needs.
