









Senior Outreach provides benefits counseling
(social security and other entitlement programs such as Medi-Cal
or Medicaid), advocacy and other services (housing and food) to
low-income older persons needing assistance getting through bureaucratic
red-tape.

The Senior Outreach program prevents premature
and unnecessary institutionalization of seniors, keeping at-risk
seniors living independently and leading self-sufficient lives.
Who are at-risk seniors?
- Isolated, frail, at-risk seniors often find
themselves alone. They are desperate but do not know how or where
to get help and the support they need.
- Many have physical or mental disabilities.
- Many are older women over 85 years of age.
- Many have language and cultural barriers
separating them from the rest of the community.
- Over 90% are low-income and must worry about
every cent.
- Many worked their whole lives and now they
may find themselves overwhelmed by just trying to find a way
to keep a roof over their heads, decent food on the table, their
utilities turned on, and a phone by the bedside in case of emergency.
- Without prompt and caring assistance, these
at-risk seniors, due to health and economic problems, face premature
institutionalization or even homelessness.
Seniors and disabled adults unsure of where
to turn for help receive hands-on, personal assistance from our
benefit counselors. Nearly 2,000 isolated seniors receive help
filling out forms, applying for benefits, arranging for medical
care or in-home help, or other services as needed to live independently.
OUTREACH workers maintain offices around the county to increase
access to isolated seniors, and go to their homes as needed.
